ADVANCE CARE PLANNING ONTARIO WORKSHOP IN A BOX

Across the province there is a need for education and awareness on Advance Care Planning for the public. To help fulfill incoming requests for presentations from various organizations, the ACP Ontario Workshop in a Box interactive training program was developed. The goal of this program is to create a network of volunteer presenters across Ontario who can deliver quality standardized Advance Care Planning presentations that are compliant with Ontario law to the public.

Thank you to Sheli O’Connor from Hospice of Waterloo Region and their Conversations Worth Having program, and Tara Cohen from the Champlain Hospice Palliative Care Program and their It’s All About the Conversations program for their respected expertise in the development of this presentation. Their willingness to give their time so generously has been very much appreciated.

 

Looking to organize an ACP Presentation?

If you are looking to secure an Advance Care Planning presentation for your community or organization, please email nav.dhillon@hpco.ca with the organization's name, contact information, approximate number of attendees, potential dates and times, length of presentation, whether there will be a question and answer/discussion period, and the make up of the audience (long term care residents, caregivers, patients with dementia, etc.)

 

Looking to become an ACP Educator?

The Educator/Facilitators are volunteers who are interested in providing Advance Care Planning presentations to the general public. For example: retired nurses or educators, hospice volunteers, and others interested in growing Advance Care Planning conversations in the community.
